Required Pre-Departure Forms
Both Wake Forest students and non-Wake students are required for submitting the following forms. Please download, complete, and turn in the forms by March 1, 2015 to the International Graduate Programs Office (Worrell 2313).
- International Insurance/ID*-iNext card: Required by Wake Forest University. Fill out the iNext card application and once your application is received and processed, you will receive an e-mail from iNext. You will then be asked to complete the rest of the process on-line (including uploading a photograph for the ID card). The iNext card will be mailed to you.
- Health questionnaire: All students must complete and sign a Health Questionnaire for participation in the study abroad programs. For information about health insurance while abroad, please visit this page.
- Assumption of risk form: All students must complete and sign an Student Assumption of Risk & Release Form. This form is a legal document, so please read it carefully.
*If you have any questions about the differences between two International Insurance ID options, please inquire with the WFU Center for International Studies (CIS) office, located Reynolda 116. You may also call CIS at (336) 758-5938.
There will be an orientation in the spring semester for each program to help prepare you for your time abroad. Additionally, you may visit WFU Center for International Studies Travel Forms website for more information about studying abroad.
